Complex Case 4: Challenging Root Canal Treatment of Calcified Lower First Molar with Rare Three-Root Anatomy

A 60-year-old female patient presented with persistent pain in a lower first molar. Clinical and radiological examinations revealed a long-standing cavity and severe calcification of the root canals, making cleaning, shaping, and filling highly challenging. Radiographs suggested an unusual root anatomy, with more than the typical two roots. During treatment, it was confirmed that the tooth had three roots and a total of four canals, a rare anatomical variation for a lower first molar.

Using advanced endodontic tools and techniques, all canals were successfully negotiated, thoroughly cleaned, shaped, and beautifully filled. The entire root canal procedure was completed in a single visit lasting approximately two hours. Following the root canal treatment, the tooth was restored with a composite restoration. The patient returned for a post-operative checkup after three days, reporting complete relief from pain and showing good clinical signs of healing.
